Barboursville,WV (AP) -- The Occupational Safety and Health Administration says it has cited Sulzer Metco for 23 violations discovered while investigating a fatal accident at the Swiss coatings company's Barboursville operation.

Osha's Charleston office director Jeff Funke said Thursday that Sulzer must act immediately to avoid another fatality.

Fifty-six-year-old worker Dwight Morrison died Feb. 25 after he was pulled into a metal lathe that OSHA says didn't have a proper guard.

OSHA also found violations including blocked or locked exit routes and inadequate fire equipment.

The citations carry proposed penalties totaling $56,250.

Sulzer spokesman Freidrich Herold says Morrison's death was tragic, but had nothing to do with the violations.

Herold says most of them have been corrected.
